b) Fully complete an OceanLED Rebate form and sign it in the space provided.
c) Mail the completed OceanLED Rebate form together with the UPC product code from each box, a serial code
from each box, and a copy of your sales receipt showing your purchase of a new OceanLED 1520, 2010, or
3010 Pro-Series Underwater Light to the address set forth in section 3 below.
d) Provide proof of Haul Out/In (Receipt Required).

WITH RESPECT TO THE ABOVE REBATES, YOUR COMPLETED REBATE FORM, UPC
CODE, SERIAL NUMBER AND SALES RECEIPT WILL MAKE UP YOUR “CLAIM” WHICH
MUST BE DATED ON OR BEFORE DECEMEBER 31, 2009 IN ORDER FOR YOU TO BE
ELIGIBLE TO RECEIVE A REBATE.

4. GENERAL TERMS AND CONDITIONS
These rebates are available only to retail customers who are residents
of the United States or Canada (but void where prohibited or restricted
by law) and may not be combined with any other offer from OceanLED.
Rebates are only available on the purchase of new Qualifying Products.
New, used, or re-furbished products that are purchased from online
sites are not eligible for these rebates.
OceanLED
will reimburse $10.00 only per foot towards the cost of Haul Out/In up
to a maximum of $400.00 against the purchase of x4 1520 Pro Series.
OceanLED
will reimburse $10.00 only per foot towards the cost of Haul Out/In up
to a maximum of $650.00 against the purchase of x4 2010 Pro Series.
OceanLED
will reimburse $10.00 only per foot towards the cost of Haul Out/In up
to a maximum of $1000.00 against the purchase of x4 3010 Pro Series.
Only one Claim per person, per household may be submitted in connection with the purchase of a Qualifying Product.
All
Claims must be complete and include all required enclosures and
information and be postmarked on or before December 31, 2009.
Incomplete Claims or Claims that are postmarked after December 31, 2009
will not be processed and are not eligible for this rebate. OceanLED is
not responsible for incomplete Claims or Claims lost in the mail. You
acknowledge that your participation in this rebate program imposes no
liability on OceanLED. In particular, OceanLED is not liable for any
consequential damages. If you return any part of your Qualifying
Product, you will not be eligible to receive the applicable rebate.
Please allow 6-10 weeks for the processing of Claims. All rebates
will be issued in United States dollars.
